If you want to eradicate an illness, the best way, according to fascists ,is simply act as if it didn’t exist.
Their justification is almost certainly as simple as “only gays and addicts get this, let it kill them”
Conservatism is sick, but elites like it.
They will become even richer due to the fact that they will reduce expenses on social development, will steal resources. But ‘regular people’ will not revolt against them, because “yes, they are right, fuck minorities, science and liberalism is bad”, i.e. they already are ignorant (and they embrace it!) and uneducated.
It’s like in any other authoritarian LMIC country, just fucking Middle Ages.
Capitalists see almost everyone, ie poors, the same way conservatives see their out groups: less than human.
Trump has literally believed his whole life (and he was taught to fervently believe this by his own father) that any sort of illness is a sign of weakness.
So, in his eyes, better to let these weak people die than waste resources on them.

Just for a little context, this program was started under W. Bush and is widely seen as his primary or sole positive legacy.
I can’t prove this is designed to aggravate the Bushes and their supporters. But who can seriously say that Trump and his henchmen wouldn’t be that petty?
Politically, this was a key proponent in creating and managing difficult alliances with African, Asian, and South American countries. China, and potentially Russia and India have been waiting for this splinter. They will flood the markets with cheaper generics and people will adapt. This will be a power play America will never recover.
